Policies
We hope this information will be helpful and contribute to a better understanding
between you and your Physician
Office Hours: Our office hours are 8:00
am to 4:30 pm Monday through Friday. The clinic is closed from 12:00 to 1:00
for lunch, so the phones are turned over to our answering service. During this
time if you feel your call is urgent you may leave a message with the answering
service. Otherwise, if you will please call after 1:00, our staff will assist
you.
Telephone Calls: We ask that you make non-emergency calls during regular office
hours when your medical records are available to us. We will return your call
as quickly as possible in the order received and generally in the same day in
which you call.
Appointments: We ask that you arrive for your appointments 15 minutes early for
any paperwork that may need to be completed. We further suggest you schedule
your follow-up appointment at the time of check out so you have a better selection
of appointment times.
Late Appointments: As we strive to have our office run as efficient as possible,
we want to be respectful of your time. If you arrive more than 10 minutes late
for your appointment, you may lose your current appointment slot and be moved
to a work-in status or possibly need to be rescheduled.
No Show Appointments: If you fail to keep a scheduled appointment, the clinic
holds the option of charging you a fee for that missed appointment.
Medication Refills: Please have your Pharmacy fax your medication refill request
to our fax line (214-363-7778). Please allow 24-48 hours for us to complete your
request.
After Hours Care: If you have an urgent matter that cannot wait until regular
office hours, please call our main number (214-345-8250) and give this information
to the Answering Service. They will contact the Physician on call who will return
your call. In the event of a life-threatening emergency, call 911. If you have
call block on your phone, please disengage this by pressing *87.
Fees, Insurance and Health Plans: We expect payment at the time of service; and
all fees and co-payments will be collected at the time of your visit.
Please bring your current insurance information to each office visit.
If
you do not have your insurance card with you, you will be expected to pay for
the
visit until the information is provided. We will file your insurance for you
if we participate in your plan. If we do not participate on your insurance plan
we will collect all fees at your visit and provide you the necessary information
so you can file your claim on your behalf.
Parking: There is valet parking offered in the front of Tower I and there is
also unassigned parking in the front of both towers. You will also find additional
parking available, at no charge, in the parking garage behind Tower II.
